What is #118? Do you have an image? And what kind of polish do you mean in #312? Personally I'd like some sort of gradient to white on the sides that overflow. I tried to make that once in css and failed, so js is needed.

I am leaning towards your single line design
Maybe, a select could differentiate between different groups of controls
E.g. elements, text formatting, paragraph formatting, alignment, etc...
The gradient to white is good
I bet we could do it with :before and :after and then use js to toggle classes to activate them

Yes, or tabs. I never seem to know under which tab something is in e.g. recent MS Word though, but then again I haven't used it much.
Yes, unless we want an animation for the gradient based on scroll position.

One other option is a permanent gradient plus some padding.
