Thanks @iangilman. You’ve got a codepen that does something like

setTimeout(function () {
    viewer.world.removeItem(oldTiledImage);
}, 1);

I was playing with it, removing the setTimeout and adding it back in. Sooo… obviously it works better with the timeout. Can u pls help me understand why? Is that because it takes longer to create (viewer.addTiledImage) than destroy (viewer.world.removeItem)?
